What is Health and Wellbeing Coaching?
Health and Wellbeing Coaches are trained professionals who work in partnership with you to help improve your quality of life. Using behaviour change techniques, they support you to set and achieve goals linked to your health, long-term conditions, or mental wellbeing.
Whether you are managing stress, coping with a health condition, or making lifestyle changes, our coaches provide personalised encouragement and support focused on what matters most to you.
1ON1
You will be offered up to six one-to-one sessions, delivered either face to face or over the phone.

Managing everyday mental health challenges such as stress, worry, or low mood

Making lifestyle changes recommended for long-term conditions (for example, diabetes or high blood pressure)

Building confidence and skills to take control of your health and wellbeing
Together, you and your coach will create a plan that is meaningful to you. Your coach will provide practical tools, motivation, and ongoing support to help you achieve your goals.
Who Can Access This Service?
Eligibility
This service is available to anyone aged 18 or over who is registered with a GP practice within the North Solihull PCN and Sheldon PCN.
If you are unsure whether your GP practice is part of this network, please contact your surgery or get in touch with us for more information.
How to Get Started
How It Works
1. Referral
Speak to your GP or another healthcare professional to be referred to the service.
2. First Contact
Your first session gives you the opportunity to discuss your needs, priorities, and goals with your coach.
3. Personalised Plan
Offer up to six sessions, you and your coach will work together to set goals and develop a personalised plan. Sessions can take place face to face or over the phone, depending on what works best for you.
4. Ongoing Support
Your coach will offer encouragement, practical resources, and strategies to help you make progress. Your plan can be reviewed and adapted as your needs change.
5. Moving Forward
By the end of your sessions, you will have the confidence, knowledge, and skills to continue making positive changes independently.
