Position: Health and Wellbeing Coach
Location:  Sheldon Primary Care Network (PCN)
Hours: Full-time, 1 role available
Contract: 2 years fixed term
Salary: £27,500 – £29,000 (dependant on qualifications and experience)

We are looking for an enthusiastic and compassionate Health and Wellbeing Coach to join the Sheldon (PCN) team.

This role is entirely based within GP practices across the North Solihull area Monday to Friday, working as part of a multidisciplinary primary care team. You will support patients to take proactive steps to improve their mental health and wellbeing by delivering low-intensity interventions, health coaching and motivational support within a primary care setting.

The Role

As a Health and Wellbeing Coach, you will provide low-intensity interventions to help patients manage their own recovery and improve their overall wellbeing. You will deliver one-to-one sessions using coaching techniques and motivational interviewing to empower patients to achieve their goals through personalised action planning. Referrals will come from within the PCN and will be across sites within the area.

Working closely with GPs, Practice Nurses, Social Prescribers, Mental Health Practitioners and other primary care professionals, you will ensure patients receive the right support at the right time. You will also work collaboratively with external health and wellbeing services where appropriate to facilitate onward referrals and integrated care.

You will be responsible for managing your own caseload, maintaining accurate clinical records and ensuring safe, effective, and patient-centred care.

Responsibilities

Duties and responsibilities include:

  • Conduct patient-centred assessments to identify areas where patients wish to make positive changes.
  • Develop personalised action plans and agree achievable goals with patients.
  • Assess and manage risks relating to patients’ mental health and wellbeing, escalating concerns where appropriate.
  • Support referral partners to make appropriate referrals into the service.
  • Develop shared care and treatment plans in partnership with patients.
  • Deliver evidence-based, low-intensity interventions.
  • Facilitate both one-to-one coaching sessions.
  • Work collaboratively with GPs and the wider primary care multidisciplinary team to ensure coordinated patient care.
  • Liaise with external agencies and services to support appropriate onward referrals.
  • Maintain accurate, timely and confidential patient records and complete all required documentation.
  • Monitor patient progress and outcomes, adapting support plans where required.
Required Skills/Experience

As a Health and Wellbeing Coach, you will hold a Coaching or Counselling qualification gained through a PCI-accredited organisation (or be willing to work towards the Health and Wellbeing Coach four-day training programme) alongside a mental health-related qualification such as an NVQ Level 2 or above.

You will have experience supporting individuals with mild to moderate mental health needs and be passionate about empowering people to improve their wellbeing.

Essential Experience

  • Using coaching approaches and techniques, including Motivational Interviewing or other person-centred approaches.
  • Delivering one-to-one sessions
  • Managing and prioritising a caseload effectively.
  • Working within a multidisciplinary team alongside healthcare professionals and partner agencies.
  • Developing shared treatment or support plans with patients.
  • Planning and organising your own workload.
  • Maintaining accurate patient records, referrals and outcome data.
  • Building positive relationships with patients, colleagues and partner organisations.
  • Working within a primary care or healthcare environment (desirable).

You may also have experience in roles such as Health Coach, Wellbeing Coach, Mental Health Support Worker, Counsellor, Psychological Wellbeing Practitioner, Mental Health Coach, Health and Wellbeing Adviser or similar.
